To unlock the Top Secret Area, you have to go through the secret exit in Donut Ghost House.
What to Do in the Top Secret Area
In this area, there is a Prize Block in the middle that contains a Yoshi Egg. The two Prize Blocks on the left both contain Fire Flowers, and the two on the right both contain Cape Feathers. This allows you to easily get a Yoshi, change to Fire or Caped Mario, and store a power-up in your Item Stock.
